DENTAL SAVINGS PLAN
No Insurance? No Problem! Dr. Hom is pleased to offer a Dental Savings Plan so that you and your family can get the dental care you need without the hassles or restrictions of insurance.
Every year, each member of your savings plan will receive 2 dental cleanings and exams with necessary x-rays, PLUS a 20% discount on many restorative and therapeutic services.
We are an in-network provider for the following dental insurance plans: Ameritas, Blue Cross Blue Shield of GA, Delta Dental, Dentemax, Guardian, Mutual of Omaha, Metlife, Aetna, GEHA, Cigna, Assurant, Humana, and United Healthcare.
As a courtesy, we will file a claim for you if all of your insurance information is received by us on the day of the appointment. Please be familiar with your insurance benefits. By law, your insurance company is required to pay each claim within 30 days of receipt. We file all insurance electronically; therefore, your insurance company should receive each claim within days of treatment.
You are responsible for any balance on your account after 30 days, whether your insurance has paid or not. If your balance is not paid within 60 days, a re-billing fee of 1/5% will be added per month until your accounts are reconciled. We will be happy to send you a refund after your insurance pays up.
Please know that we do not have a contract with your insurance company, you do. We cannot be responsible for how your insurance company handles your claims or for what benefits they pay on a claim. We will estimate your portion of the cost of treatment. We do not guarantee what your insurance will or will not do with each claim. We are also not responsible for any errors in filing your insurance. These claims are filed as a courtesy to you, our patient.
